Due to a documentation bug (the type mask is 3 bits long, not 2) the wrong
frame types were filled in: the B and P frame types were swapped.
This bug also hid a second bug: when a capture is stopped a last entry is
written into the pgm index buffer with internal type 0, denoting the end
of the program. This entry wasn't ignored, instead it was accidentally
returned to the caller as a P frame.

The driver should now pass the 'busy' state of the device to the cx2341x
module whenever controls are set or tried. -EBUSY will be returned if the
device is busy and the user attempts to modify certain 'dangerous' controls.
It concerns controls that change the audio or video compression mode and bitrates.
The cx88-blackbird and pvrusb2 drivers currently always pass '0' (not busy)
to the cx2341x, effectively keeping the old behavior for now.

To ease the conversion of the analog tuner sub-drivers into dvb_frontend
style tuner modules, we must remove the i2c_client interface.
dvb_frontend style tuner modules use i2c_transfer directly on the i2c_adapter.
This change only alters the interface between tuner.ko and the tuner
sub-drivers. The v4l2 / i2c_client interface to tuner.ko remains intact.
This patch adds inline functions tuner_i2c_xfer_send, and tuner_i2c_xfer_recv,
to replace i2c_master_send and i2c_master_recv inside the tuner sub-drivers.

The tuner module has a rather aggressive range of possible i2c addresses.
As per the specs available, it appears as if there are no 4-byte tuners that
actually use i2c addresses in the range 0x64 thru 0x6f, yet, tuner-core claims
the address range 0x60 thru 0x6f.
Allowing tuner.ko to probe these addresses can cause potential damage to
certain IR receivers, RTC chips or any other IC's that might otherwise reside
on the i2c bus using one of these addresses.
The plan is to remove these i2c addresses from the i2c address range of the
tuner module. If any devices are discovered that actually do have tuners at
one of these addresses, the newer i2c probing methods will be used to handle
those cases.
In order to collect this information and avoid any potential regressions,
the following warning has been added upon successful detection of a tuner
using an i2c address in the range 0x64 thru 0x6f:

Changing the VBI format requires a CX2341X_ENC_INITIALIZE_INPUT firmware
call. This can only be done if no capture is in progress. So return
-EBUSY if the encoder is busy.

In one case a Kbuild file would do something like:
foo-objs := a.o b.o
foo-objs += c.o
analyze_build didn't understand that the second lines _adds_ c.o to the
objects that make up foo.ko. It would think that foo.ko was made up of
only c.o. Should be fixed now.

Once the devices have been registered anyone can start changing the inputs or
TV standard before they have been initialized by the driver. This leads to
cases were the input is changed in an udev rule, but after that rule is
triggered the tail-end of the ivtv driver initialization can override
that by selecting the tuner input.
The correct sequence is to first setup the input, initial frequency and TV
standard before finally registering the video devices. This prevents any
udev rules from being triggered prematurely.

VIDIOC_TRY_ENCODER_CMD did the same as VIDIOC_ENCODER_CMD, now it no longer
touches the encoder.
Both the encoder and decoder commands did not clear the flags field of unknown
flags.

VIDEO_EVENT_VSYNC needs to tell the application which field it was that
received a VSYNC (odd/even/progressive). The vsync_field was added to the
union in video_event for this purpose.

Fix several instances of dvb-core functions using mutex_lock_interruptible
and returning -ERESTARTSYS where the calling function will either never
retry or never check the return value.
These cause a race condition with dvb_dmxdev_filter_free and
dvb_dvr_release, both of which are filesystem release functions whose
return value is ignored and will never be retried. When this happens it
becomes impossible to open dvr0 again (-EBUSY) since it has not been
released properly.

The attach code for HAUPPAUGE_HVR3000 and HAUPPAUGE_HVR1300 is exactly
the same as the code used by HAUPPAUGE_HVR1100, HAUPPAUGE_HVR1100LP, and
WINFAST_DTV2000H. So, those first two cards are added to the case block
used by the last three.

Everytime a build is started, the build system creates links to every
single source file from the v4l directory. This patch changes the
command used, to one that is about 15x faster. When building just one
file, the creating the links could take the majority of the total build
time.

Change some debug messages from printk() to dprintk().
Add KERN_WARNING and KERN_ERR level indicators to other printk()s that
lacked them.
Format printk lines with consistent ("%s/dvb: ", dev->name) prefix.
Fixed dprintk macro, which had an if with no else that wasn't protected
with a do {} while(0) block. That leads to "if(...) dprintk(); else" not
doing what one would expect.

All the radio drivers need video_dev, but they were depending on
VIDEO_DEV!=n. That meant that one could try to compile the driver into
the kernel when VIDEO_DEV=m, which will not work. If video_dev is a
module, then the radio drivers must be modules too.
