This patch fixes a few troubles on nxt200x implementation:
1) A nxt200x internal input control logic were done on both
cx88-dvb and saa7134-dvb;
2) The nx200x_set_pll_input callback were filling buffer, at the
5th position, without even testing if that buffer were filled;
3) a 5 byte buffer were written to the device, even if the
buffer were not properly initialized by calc_regs.

When a front-end is disabled, card drivers that use it are compiled with
a stub version of the front-end's attach function. This way they have no
references to the front-end's code and don't need it to be loaded.
If a card driver is compiled into the kernel, and a front-end is a
module, then that front-end is effectively disabled wrt the card driver.
In this case, the card driver should get the stub version. This was not
happening.
The stub vs real attach function selection is changed so that when the
front-end is a module the real attach function is only used if the card
driver is a module as well. This means a module front-end will be
supported by card drivers that are modules and not supported by card
drivers compiled into the kernel.

* nxt200x.c, nxt200x.h
- New frontend module that supports both NXT2002 and NXT2004.
So far, only tested on NXT2004. After testing on NXT2002, we should
deprecate the nxt2002 module, and implement this one instead on the
applicable cards.
* get_dvb_firmware:
- Added support for the NXT2004 firmware. This firmware works with both
the ATI HDTV Wonder and the AVerTVHD MCE a180.
This was originally written by Jean-Francois Thibert
* dvb-pll.c
- Fixed minimum frequency for tuv1236d. It seems that the data sheets
are wrong.
