Remove fdump tool for av7110 firmware
There's no point in this, since the user can use the BUILTIN_FIRMWARE
option to include arbitrary firmware files directly in the kernel image.
Thanks to David Woodhouse for help.

There are several Kconfig items using CUSTOMIZE. Yet, most use the English
writing CUSTOMISE. This generates lots of trouble, because people sometimes type
the Kbuild item different.
Let's standardise every occurrence using the same syntax.
The changes were generated by this small shell script:
for i in `find linux -type f`; do sed s,CUSTOMIZE,CUSTOMISE,g $i >/tmp/a && mv /tmp/a $i; done

Since:
1) FW_LOADER is defined as:
config FW_LOADER
tristate "Userspace firmware loading support"
depends on HOTPLUG
2) several V4L/DVB driver just selects it;
3) select is not smart enough to auto-select HOTPLUG, if select FW_LOADER.
So, All drivers that select FW_LOADER should also depend on HOTPLUG.
An easier solution (for the end-user perspective) would be to "select HOTPLUG".
However, live is not simple. This would cause recursive dependency issues like
this one:
drivers/usb/Kconfig:62:error: found recursive dependency: USB -> USB_OHCI_HCD
-> I2C -> MEDIA_TUNER -> MEDIA_TUNER_XC2028 -> HOTPLUG -> PCCARD -> PCMCIA ->
USB_ARCH_HAS_HCD -> MOUSE_APPLETOUCH -> USB

The ttpci Kconfig file has bugs that cause it to fail in certain Kconfig
situations. The basic problem is that it selects certain drivers, but does
not depend on the dependencies of those drivers.
See http://article.gmane.org/gmane.comp.video.video4linux/35072
Using the Kconfig file also has some annoyances. For instance one can't turn
off AV7110 support unless you go down several options and first turn off
budget-patch support. Normally user selectable drivers are not forced on like
this.
The "AV7110 cards with Budget Patch" option is disabled if "Budget cards"
isn't on. Normally a driver appears nested under a driver it depends on, but
since drivers that don't depend on "Budget cards" are between the two options,
the config programs can't display the tree correctly.
The Makefile has an issue too. Some modules, ttpci-eeprom and budget-core,
appear in the Makefile under several different config symbols. If more than
one of these symbols is on, they will get added the to list of objects
multiple times. The normal convention is to have a config symbol just the
common object(s) and have the users of the that object either depend on or
select that config symbol.
This patch fixes all these issues. ttpci-eepom is under a new config symbol,
and so is the budget-core module. The four different budget card types appear
as sub-drivers under a main "SAA7146 DVB cards" option. Turning on
budget-patch doesn't force AV7110. Drivers using SAA7146_VV have the
necessary VIDEO_DEV dependency, so that it isn't possible to select SAA7146_VV
without V4L being on.

A number of drivers selected DVB_PLL when they did not need it, and some
that did need it did not select it.
The DVB_PLL option is given a name and help text, so that it will show up
in the config menu. DVB_PLL support can be turned on if an out-of-tree
driver needs it.
The standard dvb fe customization support is added to dvb-pll.h. Since
all modules which select DVB_PLL do so unconditionally, it is not
possible to turn dvb-pll off when an enabled module selects it, unlike
most of the other frontend/tuner drivers. This is because the users of
dvb-pll have static references to dvb-pll symbols other than the attach
function. If these references are removed, then dvb-pll will be
disablable as the other frontend/tuner drivers are.

This converts the budget-ci driver so that it uses ir-common for some of its
IR processing. In particular, the keymap for the Nova-T (sub 13c2:1011) is
switched to the Hauppauge grey/black keymap, of which the keys on the
supplied R808 remote control form a subset.
The old budget-ci keymap is moved to ir-keymaps.c and is used for other
remotes.
The debounce logic for buggy remotes (i.e. Zenith) is made conditional the
new debounce parameter and defaults to off (so that repeat keypresses aren't
ignored for all working remotes).
Some parts are based on Darren Salt's dvb-ir patchset.

2.5.x:
- exchanged the Kconfig files for "ttpci" and "ttpci-budget"
- removed the "SAA7146 based" from the card descriptions, but put
it to the remark some lines above
saa7146:
- simplified the probe(), attach(), detach() logic, since all
dvb devices can be identified by their subvendor/subdevice ids
av7110:
- added two wait_for_debi_done() calls in gpioirq(). (All other
IER_ENABLE() calls are preceded by wait_for_debi_done())
budget:
- budget.c has been split up into budget.c and budget-av.c. The latter
is for budget cards with analog video (av) inputs. Definitions are
now in budget.h. budget.c exports various functions that are used by
budget-av.c, too. The av code is currently missing, though, and needs
to be added.

(tested on 2.4.18 and 2.5.53 -- the budget driver is
untested, Holger will test it the next week)
saa7146:
- completely separated the saa7146 driver into a core-driver (saa7146.o) and
a video/vbi part (saa7146_vv.o). this allows you to build the budget driver
without av7110 and videodev bloat (see below)
The following thinks have been approved by Holger Waechtler:
av7110 (now ttpci):
- for 2.5.x: removed the av7110 directory and put everything into ttpci.
- removed all budget card support from this driver
- compile and build fixes for the saa7146 changes
ttpci-budget (new):
- for 2.5.x: created a new ttpci-budget directory, added a new driver for
the budget cards
mxb:
- compile and build fixes for the saa7146 changes
Changed the build and compile files for both 2.5.x and build-2.4 of course...
