Rename the flac combined plugin sources so that they are listed together in a by-name listing.

rename : src/combined/demux_flac.c => src/combined/flac_demuxer.c

Reduce size of flac_decoder_t structure: output_mode and output_sampling_rate are not used; sample_rate, channels and bits_per_sample can be local to flac_decode_data on the STDHEADER codepath.

Move the flac plugins sources into the combined directory, like wavpack.
