xine-lib/src/libspucc/cc_decoder.h, branch vdr-xine-version-802

Move all the SubPicture decoders together in the spu_dec directory. Remove superfluous xine_ prefixes from source files.

Yes, this _is_ the big metronom update, metronom now depends on disc_off information in discontinuities.
