As pointed by Marcin Slusarz <marcin.slusarz@gmail.com>, the warns happens
because CONFIG_DVB_MAX_ADAPTERS depends on CONFIG_DVB_CORE, and there are
valid configs where DVB_CORE is not selected.
This causes such warnings, for every V4L and common drivers that may or
may not be compiled with DVB support:
drivers/media/dvb/dvb-core/dvbdev.h:36:2: warning: #warning invalid CONFIG_DVB_MAX_ADAPTERS value
We can safely remove the warning.

headers: smp_lock.h redux
* Remove smp_lock.h from files which don't need it (including some headers!)
* Add smp_lock.h to files which do need it
* Make smp_lock.h include conditional in hardirq.h
It's needed only for one kernel_locked() usage which is under CONFIG_PREEMPT
This will make hardirq.h inclusion cheaper for every PREEMPT=n config
(which includes allmodconfig/allyesconfig, BTW)

Implement dynamic minor allocation for DVB, to allow more than four
devices of the same type per adapter, based on drivers/usb/core/file.c.
Add a new config option, DVB_DYNAMIC_MINORS, to make use of this
feature, which defaults to no for backwards compatibility.

This add frontend R/W mutual exclusion.
Prior to this point in time it was possible to open both
frontends simultaneously which an MFE card cannot support.
In order to stop this, a delayed open is performed which
has the following function:
Return EBUSY after a configurable amount of time
if a frontend is unavailable due to the other being
in use.
Only allow opening of a frontend if the kernel thread
of the other has stopped.
This solution was chosen to allow switching between
frontends to work as seamlessly as possible. When both
frontends are actually opened simultaneously then one
will only open, but if quick switching is performed
between one of many then the new open will succeed in
a clean fashion rather than interrupting a kernel
thread.

The adapter_nr module options can be used to allocate static adapter
numbers on a driver level. It avoids problems with changing DVB apapter
numbers after warm/cold boot or device unplugging and repluging.
Each driver holds DVB_MAX_ADAPTER long array of the preferred order of
adapter numbers.
options dvb-usb-dib0700 adapter_nr=7,6,5,4,3,2,1,0 would result in a
reversed allocation of adapter numbers.
With adapter_nr=2,5 it tries first to get adapter number 2 and 5. If
both are already in use it will allocate the lowest free adapter number.

Due to several internal API changes on kernel, kernel backward
compatibility were lost. Basically, compat.h should be the last include
for it to work properly.
This patch basically reorders kernel headers to allow backward compat to
work fine.
Also:
Some includes were added after some non-include macros, on old drivers.
Better to keep all includes at the beginning of the files.

Currently in /sys/class/dvb/dvbX.demuxY/ we have:
dev
uevent
With the patch, we have (for a PCI DVB device):
dev
device -> ../../../devices/pci0000:00/0000:00:1e.0/0000:03:0d.0
uevent
So userspace tools can (finally) work out which physical device a DVB adapter
refers to. Previously you had to kinda look through dmesg and hope that it
hadn't been dumped out of the buffer. This makes debugging a lot easier if
the system has been up for a long time!
This is done by adding an extra 'struct device *' parameter to
dvb_register_adapter(). It will work with any kind of standard
linux 'device'. Additionally, if someone has an embedded system which does
things differently, they can simply supply 'NULL' and the behaviour will be
as before - the link will simply not appear.

"kernel_thread_setup()" (now: dvb_kernel_thread_setup()) and
dvb_usercopy() function.
Because of the fact that we want to separate the dvb subsystem, it's
sometimes necessary to have some wrapper functions (for example
dvb_delay() or dvb_kernel_thread_setup()) to encapsulate linux
kernel facilities that can't be expressed as a one-liner.
The place for such functions is "dvb_functions.c". By this, we can
get rid of "dvb_compat.h" for 2.5 completely. Hooray!
I hope I did not break everything again, some cleanup is expected for
the build-template stuff, though.

- dvb: removed the compat module from the 2.5.x build process, added #ifdefs
around the inclusion of compat.h. this was necessary because having compat
code in the newest 2.5.x kernel is awkward.
- dvb: move around of dvb_usercopy again. because I removed the compat module
from the 2.5.x build process, dvb_usercopy had to be moved to another file
- dvb: change MOD_*_USE_COUNT to module_get/module_put in various files,
but I'm not sure if I did this right. please review these changes.
- dvb: removed the crc32 code from dvb_demux.c, because it's in compat.c, too.
- dvb: removed some cruft from dvd_i2c.c, which is present in compat.h
- all files: apply small fixes to reflect the new location of some header
files
