* fix the realloc-bug introduced by me and found by Frantisek
* since plugin catalog assumes a limit on plugin count, we should actually
enforce this limit on plugin loading
* reduce DECODER_MAX from 256 to 128, because PLUGIN_MAX is already 256
and having only decoder plugins is unlikely
